FRED M. RUTAN OUTSTANDING SERVICE AWARD

ELIGIBILITY:

1) Any physical therapist or physical therapist assistant who is or has been a member of the FPTA and has performed outstanding service to the profession of physical therapy within the state of Florida.

2) Has demonstrated continued interest in and cooperation with the Chapter.

3) Has participated in activity and been a dominant force in activities which have significantly affected Chapter
and professional growth and development.

4) Has made contributions of a truly outstanding nature.

NOMINATION:

Nomination must be made by a Chapter member and is acceptable at any time.
Nomination must be in writing and include:

*Name *Occupation
*Address *Biographical Data
*Phone Number *Statement containing documentation of activities and contributions which qualify them for nomination.

SELECTION:

The Executive Committee is responsible for evaluation of nominees. The Executive Committee will submit nominees to the Board of Directors. Final selection of recipients shall be made by the Board of Directors. A 3/4 majority vote will be required.

PRESIDENT'S SERVICE AWARD

ELIGIBILITY:

Any Physical Therapist of Physical Therapist Assistant who is or has been a member of the FPTA and has performed or provided exceptional service to or on behalf of the Florida Physical Therapy Association.

NOMINATION:

Nomination must be made by a Chapter member and is acceptable at any time.

Nominations must be in writing and include:

Phone Number
Professional Status
Statement containing documentation of activities and contributions which qualify them for nomination.

SELECTION:

The Executive Committee is responsible for evaluation and final selection of awards.

RICK SHUTES COMMITTEE SERVICE AWARD

ELIGIBILITY:

Any Physical Therapist or Physical Therapist Assistant who is or has been a member of the FPTA and has performed or provided exceptional service to or on behalf of the Florida Physical Therapy Association as either a committee chair or as a committee member.

NOMINATION:

Nomination must be made by a Chapter member and is acceptable at any time.

Nominations must be in writing and include:

Address
Phone Number
Professional Status
Statement containing documentation of activities and contributions to the committee, and the
accomplishments of the committee, which qualify them for nomination for this award.

SELECTION:

The Executive Committee is responsible for evaluation and final selection of awards.
